Job Roles

  • Reviews referrals as they are received from Pediatrics and other referring partners for assigned site(s).
  • Initiates contact with families in a timely manner to thoroughly explain the reason for the referral to the parent; and addresses any inquiries or concerns the parent may have.
  • Assesses any familial needs and/or barriers to engaging in services. Provides information about the importance and benefits of developmental services for the child and explains next steps.
  • Schedules the child for the in-depth developmental and social emotional screening (ASQ and ASQ- SE)
  • Administers in-depth screening and communicates results, with recommendations about next steps (e.g., additional referrals to other CBOs, such as Regional Center; referral for further assessment, and/or to specific specialty consults or evaluations for OT, SL or PT or IMH).
  • Investigates reasons for missed appointments and identifies new or unexpected barriers to attending the appointment, problem-solves in partnership with caregiver, and reschedules the appointment. Identifies trends and communicates to referring medical provider(s), or community partners per department procedures.
  • Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards. Assists in preparing reports as required.
  • Optimizes use of time by conducting referral case management and/or scheduling additional patients. In partnership with support staff, the DSS is responsible for maintaining their own schedules full.
  • For ongoing scheduling of appointments, works in collaboration with support staff assigned to the program. Ensures that children and families are connected to the next service within a maximum of 30 days.
  • Monitors the response and service access timeline for each child referred.
  • Performs other duties as assigned.

Education/Certifications/Licenses/Registrations

  • Bachelor's degree in health and human service areas, including Child Development, Psychology, Public Health, Early Childhood Education, Social Work, Human Development, or a related field.
  • Ability and means to travel as needed in a timely manner within San Diego County. DRIVER REQUIREMENTS: Requires an active Class C California driver's license, proof of liability insurance at $100,000, on vehicle used. No more than 2 points in past 12 months; No suspensions in last 2 years for moving violations; No DUI, reckless or felony Driving within 5 years. No license revocation in 7 years.

Experience/Specialized skills (including Language)

  • Minimum of 2 years of experience in working in positions related to child health and/or development, or related area required.
  • Experience in oversight of services which support child development, health, or education related programs/services helpful.
  • Cultural and linguistic sensitivity to populations served, experience with medically underserved populations.
  • Knowledge about and experience with diverse populations
  • Experience in working with families with young children.
  • Knowledge of child development (milestones; concerns, services, etc.), ages 0-5 years
  • Prior experience in case management responsibilities consisting of screening families for needs, and knowledgeable about local resources for families desirable.
  • Possess excellent verbal, written and listening skills.
  • Strong analytical and problem-solving skills
  • Bilingual English/Spanish

$25.63 - $32.83

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
